How to Share Your Feedback or Make a Complaint

Online – Use the simple feedback form below
Phone – Call us on 02 4623 8677 during business hours
Email – Send your feedback to info@ampeservices.com.au
In person – Speak directly with any member of our team at our office or at one of our homes

You do not need to give your name if you prefer to remain anonymous, although providing contact details helps us follow up and resolve any issues more effectively.

When you share feedback or raise a complaint, we promise to:

  • Acknowledge your message within two business days
  • Treat your concerns with respect, confidentiality, and without any negative impact on the supports you or your family receive
  • Investigate fairly and thoroughly, involving the right people to understand the full picture
  • Provide a clear written response outlining what we have found and any actions we will take
  • Follow up with you to make sure the outcome meets your expectations

We are fully committed to the NDIS Code of Conduct and view every piece of feedback as an opportunity to strengthen the trust and quality of care we deliver.
